DOL releases updated FMLA forms for immediate use
Many employers have expressed concern because the model FMLA certification and notification forms available from the DOL website still carried an expiration date of December 31, 2011. At last, the DOL has released new model forms, but those forms still are missing some critical information. See the attachment below for links to the forms and a discussion of the material that is still missing.
